Northeast Health was formed in 1995 by the merger of Samaritan Hospital and The Eddy, and joined by Albany Memorial Hospital in 1997 and Sunnyview Rehabilitation Hospital in 2007. We're proud of our Schools of Nursing which, for more than a century, have been educating and preparing students to become skilled and competent register nurses (RNs) and licensed practical nurses (LPNs). Adult Day Services Eddy DayBreak is an innovative adult day service that helps improve quality of life for seniors who still live independently in the community, or who are cared for by family members. DayBreak programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home caregiving so they can fulfill work or other obligations. DayBreak services include: skilled nursing and preventive medical care assistance with bathing or showering social work services and care coordination rehabilitation services, including physical, occupational & speech/language therapies podiatry nutritional counseling and help with special dietary needs a variety of engaging and stimulating activities. Eddy DayBreak programs feature medical, social and dementia-specific models to accommodate different levels of need. Eddy DayBreak at Rensselaer -- an award-winning social day program that provides a safe, supervised environment that enables older clients to maintain an active lifestyle while still being able to enjoy the comforts of home. Winner of the 1999 New York State Intergenerational Network Program Award, this program also features creative arts therapy with on-site music, art and dance therapists to work with seniors. 50 Herrick St., Rensselaer, NY (518) 436-4302 Eddy DayBreak at Eddy Memorial Geriatric Center -- The first adult day service in the Capital Region, this medical model program has been making a difference in the lives of its participants and caregivers in the community for more than 20 years. Eddy DayBreak at North Greenbush -- This medical model program is located at the Stepping Stones II Building in Van Alstyne Corporate Park, in the same building as several child care and afterschool children's programs -- making it an ideal setting for intergenerational programs to benefit both children and seniors. 300 Valley View Blvd., North Greenbush, NY (518) 286-1986. Eddy DayBreak at Cohoes -- This medical model is the largest adult day program in the Capital Region. Eddy DayBreak at Marjorie Doyle Rockwell Center -- This medical day program features a caring environment designed especially for older adults experiencing memory loss. The program offers a supervised environment for adults with dementia; assistance with personal care or other activities of daily living; monitoring of medications; and care management. Nurses work closely with the participant's personal physicians to address medical concerns. A range of recreational and social activities are designed to accommodate various levels of cognitive and physical functioning. 421 West Columbia St., Cohoes, NY (518) 238-4152 Eddy DayBreak sites in Troy, North Greenbush, Cohoes and Marjorie Doyle Rockwell Center in Cohoes accept Medicaid, while Eddy DayBreak in Rensselaer participates in several Medicaid waiver programs.
